FILING A CLAIM

In the event that you are assisting an employee to file a claim, please help them complete and return the NCPERS Claim Form for Death Benefits.
  • Claim Form: PDF

    Death claims are usually paid within 4-6 weeks from receipt of complete claim information. Incomplete information will delay the claim.

    Benefit checks under $5,000 are paid to the beneficiary for the full amount in a lump sum. Benefit checks over $5,000 are paid utilizing a personalized interest-bearing account for beneficiaries of group life insurance.
